These treatments are widely available, aggressively marketed, and often presented as the obvious first step when someone starts to feel unlike themselves. And for some patients, in the right context, some of these treatments absolutely have a place.

The other side of the fork.

The Problem with High-Energy "Melting" Treatments

These treatments create controlled thermal injury to stimulate tightening and collagen production, but repeated or aggressive treatments can sometimes lead to tissue changes beneath the skin. As a surgeon, I occasionally see patients with more fibrotic, more fragile, less pliable tissue after multiple high-energy treatments, which can make surgery more complex. It can also decrease your result potential.

My philosophy is simple: these technologies have a place, but they should be used thoughtfully, conservatively, and by experienced providers who understand both facial anatomy and long-term tissue health.

The Problem with Excessive Filler

Filler, too, has cumulative effects that aren't always discussed openly. Hyaluronic acid filler, when used judiciously and appropriately, can be a lovely complement to surgical rejuvenation or add volume in the properly selected patient. But many patients arrive in my consultation room having accumulated significant volumes of filler over years of treatment — filler that has migrated, built up in unexpected areas, or created a heaviness in the tissues that is visible and difficult to reverse.

Addressing this before surgery often takes time and additional treatments. And it means that before we can move forward toward your refresh, we sometimes have to take a few steps back — which costs both money and requires patience.
