In order to correctly and efficiently correct the aging effects to the midface, one must truly understand the anatomy and the changes taking place. The face of youth is abundant in facial volume- high cheeks, small nasolabial folds, no shadows.

During your private one-on-one consultation, Dr. Robb Jr. will assess your areas of concern, discuss your medical history.

There are many different ways to restore volume to the midface and each has its own distinct advantage over the others.

  • Injectable fillers are a safe and low-risk temporary way to increase facial volume by smaller increments that are temporary. “See how you like it” – many patients choose more permanent options after seeing their new look
  • Midfacial Implants – these are implants that are placed under the skin and soft tissue layers of the face. They are made out of silicone and silastic synthetic materials. The placement of these is quick, precise, and done with very little discomfort. The results are immediate and predictable. If the results don’t meet the desired effect, they are easily removed or exchanged for a different one.
  • Fat injection (autologous fat) – This method uses your own fat tissue (harvested from elsewhere on your body) and reimplants the fat into the area requiring volume.


Dr. Robb Jr. believes that educating our patients is one of the most important aspects of preparation for the procedure.

Adding volume to the midface often provides a subtle but significant change in facial shape. Heart-shaped faces full of volume in the cheeks and tapers down to a tight jawline is analogous with beauty, young, and vibrant.

Stopping all blood thinners including NSAIDS (advil, motrin, aleve, etc) 1 -2 weeks prior to the procedure will decrease swelling, oozing, and operative time.

Midfacial implants can be placed under local anesthesia in the office or in the operative room under a deeper plane of anesthesia. The procedure is well tolerated by both methods.

The incision is placed in the mouth so that no scars are left on the face. For the first few days after the procedure, soft foods will be better and more comfortable.

It’s always a great idea to pick up your post-operative medications prior to the day of the procedure.

Dr. Robb Jr. will meet with your prior to the procedure to answer any questions you may have. You will also finalize the surgical plan regarding the amount of projection desired and the exact location of the projection.

Markings and measurements will be made to ensure exact placement to create symmetry.

The incision is hidden inside the mouth, a small pocket is created in front of the bone for the implant to slide into. Once the projection is in the perfect location, they are secured into place and washed with an antibiotic solution to prevent infection. The intraoral incision is closed and the procedure is complete.


Swelling and some mild bruising are normal. Dr. Robb Jr. provides pain medication to minimize discomfort during the recovery period.

Rotating cold compresses and decrease the swelling and keeping the head of bed elevated also works to help decrease swelling and pain.

A soft diet is preferred for the first several days after surgery. Bruising and swelling will subside in the first several days. Antibiotics will prevent infection and pain medication with be prescribed to minimize discomfort.

Fat Transfer is very well rated by patients. Restoring volume and fullness to the midface and creating “high defined cheeks” provides excellent enhancement of the face.

Using one’s own fat or the implant technique is permament, the implants can be adjusted or removed while fat is not as easy to remove.

Using autologous fat for midfacial rejuvenation is an excellent option with one exception. Much of the fat injected (up to 50%) will dissolve and not provide volume and projection. Because of this, some patients love their faces shortly after surgery but request more fat to be placed to achieve significant results.
